This article will focus on uncertainty in perinatal medicine. Sources of uncertainty include not only the complexity of premature physiology and related disease mechanisms, but also the lack of proper assessment of many perinatal technologies in terms of their safety and effectiveness....We will illustrate various issues involving uncertainty in perinatal medicine by means of case studies of two perinatal technologies -- transcutaneous oxygen monitoring and neonatal ECMO [extracorporeal membrane oxygenation] -- and will analyze our observations in terms of a proposed ethics of evidence. We will conclude by placing these issues in the broader context of contemporary perinatal medicine.en
