Abstract
How should the record of a clinical case consultation be crafted? We begin by surveying some of the various approaches to organizing and recording information in the medical record, seeking lessons for ethicists from the medical model. We then survey some of the many needs which an ethics consultation note might serve, and how these purposes might influence the structure and content of the note. We conclude by suggesting some guidelines for constructing notes concerning the more common health care ethics consultations.
