Discussions About the Use of Life-Sustaining Treatments: A Literature Review of Physicians' and Patients' Attitudes and Practices

...[C]ommunication with physicians can take various forms, including written advance directives such as living wills or direct discussion. The usefulness of living wills is limited by their vague language and by their applicability to a narrow range of clinical situations. Common sense suggests that the most effective way for patients to communicate their wishes about the use of LST [life-sustaining therapy] is by discussing these issues directly with their physicians. However, common sense may not always prove to be correct. The purpose of this article is to review what is known about discussions between patients and physicians about the use of LST. Specifically, we will address the following questions: (1) Do patients and physicians discuss LST? (2) Do patients and physicians want to discuss the use of LST? (3) When do patients and physicians want to discuss LST? (4) What are the reasons patients and physicians do not discuss LST? (5) How well do physicians understand patients' preferences for LST after discussion? (6) What are the best ways to discuss the elective use of LST to insure that patients clearly communicate their preferences?
